We have diagnosed a case of dermal cylindroma of scalp in an elderly female patient by fine needle aspiration cytology of the lesion and further confirmation was done by histopathology.

Multiple cylindromas may form a “turban tumors” in the autosomal dominant Brooke–Spiegler syndrome. Malignant dermal cylindromas are very rare. There are few cases of malignant transformation of dermal cylindromas in the literature.

Poromas may arise on any skin surface but those with eccrine differentiation are most commonly found on the palms, soles and scalp. Cylindromas are uncommon benign adnexal tumor.

An unusual case of malignant cylindroma of the scalp arising in a 79-year-old white female with multiple cylindromatosis is presented. The tumor apparently arose from a cylindroma and had features of spiradenoma. Multiple cylindromatosis is an uncommon hereditary autosomal dominant disease, which is characterized by multiple skin adnexal tumors like cylindromas and trichoepitheliomas and Cylindroma was first described by Ancell in 1842. [1] However, the term cylindroma was coined by Billroth in 1859.
